33 organizations form the Puerto Rico Healthcare Coalition (PRHC) submitted today a Community Letter to CMS, requesting three concrete and urgent actions for the Medicare Advantage program in Puerto Rico:


  1. Establish a reasonable benchmark floor;

  2. Implement a minimum Average Geographic Adjustment of 0.70 t; and

  3. Work with Congress to extend essential federal programs (MSP, LTSS, LIS, SSI, SNAP) to Puerto Rico


With 96% of Medicare-eligible older adults in Puerto Rico enrolled in Medicare Advantage, the stability of the program is vital to the Island’s healthcare ecosystem. Yet the federal payment formula continues to rely on data that do not reflect Puerto Rico’s reality, creating imbalances that threaten access to physicians, specialists, and essential health services—even though Puerto Rico maintains one of the highest concentrations of 5-Star plans in the nation.


This effort seeks to protect access, quality, and continuity of coordinated care for thousands of Medicare Advantage beneficiaries in Puerto Rico. We remain united, raising one voice in support of a stable, functional, and sustainable healthcare system for our people.
